Completed student application form.(attached)
Student QID (2 sided) and Copy of passport.
Copy of the birth certificate.
Two passport size photos.
Previous school report card signed and stamped by the school.
Copy of the student’s health record and Hamad Medical health Card.
Completed recommendation form filled and signed by the previous school in a sealed envelope or to be send to registration@hasqatar.com
Copy of Father’s QID (2 sided) / Copy of Mother’s QID (2 sided)
Copy of Father’ passport / Copy of Mother’s passport
Work certificate for the guardian
Copy from the national address from Metrash2/Blue plate
